Hello, 

My name is Kristy DiLoreto, and I am a genetic counseling graduate student at Case Western Reserve University. You are invited to participate in a research study about your referral practices and barriers to genetic services for patients who have pancreatic ductal adenocarcinoma (PDAC). You are invited to participate in this research because you are a nurse practitioner who is a member of The North Alabama Nurse Practitioner Association. This study is being conducted by researchers at Case Western Reserve University.

 

You are eligible to participate in this study if you:

  • Are a non-genetic physician or advanced practice provider
  • Care for patients with pancreatic ductal adenocarcinoma or patient with a family history of pancreatic ductal adenocarcinoma

 For most participants, the survey should only take about 10 minutes.

Taking part in this survey is voluntary. By completing this survey, you will be consenting to this study.

The study is confidential. All of your answers will be kept strictly confidential. Responses will not be shared with anyone outside of the research team. Any report of this research that is made available to the public will not include individual information by which you could be identified.

 

During the survey, you will have the chance to be entered in a raffle to win one of two $50 Amazon electronic gift cards.
